ESSENTIAL JOB FUNCTIONS
Minimum Requirements:
Licensed Practical Nurse with experience preferably in a mental health setting.
Applicants must have a reliable vehicle and be willing to utilize it on a regular basis
Duties include:
- Complete mental health consumer nursing assessments and related paperwork
- Assist in identifying consumers with complicated medical or psychiatric needs and provide the appropriate level of nursing care within the scope of the program.
- Respond to consumers specific needs such as educating and supporting consumers regarding medical conditions and assisting in monitoring more complicated medications.
- Assist in identifying, responding to and following up on any medication variances that occur.
- Complete, verify and update monthly medication observation reports and lists.
- Inspect medication boxes and charts to ensure accuracy and complete medication reconciliation process.
- Participate in discharge planning when consumers enter an inpatient psychiatric unit or other facility.
- Obtain discharge papers and medical records and provide follow-up care to consumer upon discharge from any facility.
