As a Full Time PrEP LPN/Navigator at Katy Trail Community Health, your day-to-day activities will be both varied and rewarding. You will primarily be responsible for conducting assessments, educating patients on PrEP, and providing emotional support throughout their healthcare experience. Expect to collaborate closely with healthcare providers, ensuring seamless patient navigation and care coordination.
Your daily schedule will typically run Monday to Friday, from 8:00 AM to 5:00 PM, with occasional weekends and evenings to accommodate patient needs and community outreach activities. You will engage in outreach efforts, participate in community events, and help promote awareness of PrEP services. Daily responsibilities will also include maintaining accurate patient records and following up with clients to monitor their health outcomes.
