Case Manager, Mobile Crisis
POSITION OVERVIEW
POSITION SUMMARY
Under the direction of the Mobile Crisis adjunct Director/Site manager, the Case Manager will provide case management services in order to support the crisis intervention and follow-up plan for each client. These services will be implemented using a strength-located, trauma informed, systemic perspective, and will include coordination of services with existing treatment providers and linkage to additional community located therapeutic resources and supports. ESSENTIAL tasks AND RESPONSIBILITIES include the following. Other tasks may be assigned. Case Management Functions: »Assist the Mobile Crisis leadership abilities with coordinating case planning. »Consult with other community agencies to identify potential resources for case management services »Per existing protocols, coordinate and follow up with Crisis Respites »Provide back-up on crisis calls with a Crisis Team Clinician on an as needed basis »Provide consultation to inter-agency service providers and to community service providers. Maintain appropriate liaisons & relationships with external & internal providers Case Management monitoring and follow up: »Monitor client and provider satisfaction with the services provided. »Record all client-related communication, interactions, and contacts in progress notes. Data Management and Administrative Tasks: »Assist administrative staff with entering information into the database, including entering demographic and required fields, ensuring that data is entered in a timely manner. »Routinely run reports to monitor timelines, referral processing, county usage, dispositions, interventions, results, cost savings and diversion statistics, alerting administrative staff and Program Director/manager to critical concerns. »Monitor records for completeness prior to case closure, alerting Program Director/manager to records that are not in compliance. »Assist IT in updating or changing database needs as needed. »Forward aged out records per agency guidelines to the Records Coordinator. »Assure that mail is delivered to/from program staff at least once daily. »Maintain sufficient supplies and building related operational needs for program operations, submitting requisitions to Program Director as needed. »Perform other tasks as assigned and approved by Program Director. Record Keeping: »Complete all record keeping and reporting requirements connected with program as required, including progress notes, service utilization and monthly statistics and reports. »Ensure confidentiality in all clinical, fiscal, and administrative matters, including monitoring records cabinets for security and proper sign-out procedures and following release of information guidelines. managerY RESPONSIBILITIES this job offer has no managery responsibility.
